Job Type: Fixed Term Contract, 1 year Maternity Cover

Our client is a dynamic, multi-award-winning teaching Partnership, serving patients in the community for more than 30 years, in the Woodberry Wetlands region looking after circa 12,000 patients.

The Role

The post holder is responsible for the delivery of care management to the whole practice population and will undertake tasks and duties delegated by the GPs and practice nurse team. They will work collaboratively within the practice team to meet the needs of patients and supporting the delivery of policy and procedures. Main Duties:

You will work within your scope of clinical practice and:

Undertake, record and follow guidelines for the tasks for which you have received appropriate training including:

  • Phlebotomy
  • Taking and monitoring blood pressures
  • New patient and NHS health checks
  • Weight management
  • Ear syringing
  • Basic wound care
  • Administration of influenza/pneumococcal vaccines/B12’s
  • Spirometry
  • Long term condition reviews
  • Health promotion Home visiting
  • Chaperoning duties
  • Prepare and maintain environments and equipment before, during and after patient care interventions
  • Give accurate Participate in the administrative and professional responsibilities of the practice team
  • Ensure accurate and legible notes of all consultations and treatments are recorded in the patients notes
  • Ensure the clinical computer system is kept up to date with accurate details recorded
  • Ensure accurate completion of all necessary documentation associated with patient health care and registration with the practice
  • Ensure collection and maintenance of statistical information required for regular and ad hoc reports
  • Monitor the vaccine/cold chain storage and record information in accordance with practice policy
  • Attend and participate in Practice meetings as required
  • Assist with the recall system
  • Assist in seasonal and special projects e.g. flu campaign as required
  • Assist with vaccine re-stocking and stock rotation as require
  • Demonstrate sensitive communication styles to ensure patients are fully informed and consent to treatment
  • Communicate effectively with other team members
  • Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating
  • Recognise people’s needs for alternative methods of communication and respond accordingly
  • Apply practice policies, standards and guidance
  • Discuss with other members of the team how the policies, standards and guidelines will affect own work
  • Participate in audit where appropriate

Skills and Qualifications

Essential:

  • At least 1 year’s recent experience of working in Primary Care
  • Good standard of literacy and numeracy
  • Willingness to undertake further training/ qualification
  • Clinical skills – including phlebotomy, taking and monitoring blood pressures and new patient health checks
  • Demonstrates a basic understanding of the needs of the patient and the role of the HCA.
  • Ability to deal tactfully and empathetically with distressed or anxious patients and carers

Desirable:

  • Minimum of 2 years’ experience of working in a primary care environment
  • Care Certificate
  • Evidence of continuing personal and professional development
